We are looking for a Social Services Director to join our
team of dedicated healthcare professionals! The SOCIAL
SERVICES DIRECTOR plans, organizes, develops, and directs the
overall operation of the Social Services department to ensure all
medically-related emotional and social needs of patients are met in accordance
with all federal, state and local regulations, established procedural
guidelines and as directed by the Administrator.
Location: Brunswick, GA
The Responsibilities (not all inclusive):
- Applies standards of professional
Social Service work practice
- Demonstrates knowledge and
respect for the rights, dignity, and individuality of each resident in all
interactions
- Demonstrates competency in the
protection and promotion of and advocacy for residents’ rights
- Responds to resident behavioral
and psychiatric issues by completing behavioral and psychosocial
assessments, providing treatment recommendations and making referrals to
appropriate mental and behavioral health providers
- Coordinates discharge plans
with residents and families
- Assists patients and families
with advance directives in accordance with Federal regulations.
- Resolves and tracks grievances
- Completes psychosocial
assessments of residents
- Flexible work schedule
- Manages Social Services
department staff to ensure patient coverage and satisfaction
- Contact the resident or legal
representative and invite to care plan meetings to participate in the
evaluation of the resident
- Coordinates ancillary
appointments as necessary
The Requirements:
- Two (2) years' experience in health-care social work
preferred
- Must be able to work weekends and holidays as needed
- Ability to meet all health requirements, including a
drug screen
- Ability to pass a criminal background check
- Prior long-term care experience is preferred but not
required
