Clinical Nurse Liaison

It is our mission at Summerhill Senior Living Community, to keep all of our sweet vulnerable residents safe! We are seeking a dedicated, and qualified Clinical Nurse Liaison! The ideal applicant will be hardworking, reliable, detail-oriented, dynamic, and high-motivated.  

As the Clinical Nurse Liaison for Summerhill Senior Living Community, you will work closely with the healthcare community and elderly population to promote our services and increase census.

Responsibilities (not all-inclusive)

· Screen referrals for clinical appropriateness, aligning each case with CMS skilled-level criteria, PDPM clinical categories, and the facility’s specialty programs. Assist with coordination of care with admission coordinator and corporate clinical nurse in the referral process.

· Coordinate with hospital discharge planners, physicians, and therapy leadership to move eligible patients from referral to bed assignment, supporting census growth and reducing hospital length-of-stay penalties.

· Assess admissions from the hospital to ensure they are clinically appropriate for discharge, helping decrease rehospitalization rates.

· Facilitate comprehensive admission documentation by guiding families through consent forms, payer eligibility paperwork, and HIPAA disclosures; ensure 100% compliance with federal and state skilled-nursing regulations.

· Serve as the primary clinical resource for marketing in the community with community partners, translating clinical pathways into clear, patient-friendly guidance.

· Liaise with local hospitals, physician practices, and community organizations through targeted outreach and event participation to strengthen brand visibility and support census growth.

  • Strong Communication Skills: The ability to clearly and compassionately explain services, eligibility requirements, and care pathways to patients, families, and referral sources is crucial. A Clinical Nurse Liaison must be skilled at both clinical explanation and non-clinical conversation, ensuring clarity and trust at every interaction.
  • Relationship-Building Mindset: Establishing long-term professional relationships with hospitals, physicians, case managers, and community organizations is key to sustaining referral pipelines. A liaison should be approachable, dependable, and proactive in outreach efforts to maintain consistent engagement with partners.
  • In-Depth Clinical Knowledge: A strong foundation in skilled nursing, rehab, long-term care, and memory care allows the liaison to assess clinical appropriateness confidently and advocate effectively for admissions. This ensures that the facility admits residents who are best served by the available levels of care.
  • Marketing Acumen with a Compassionate Approach: Representing Summerhill’s services authentically while highlighting its strengths (e.g., short-term rehab, long-term care, assisted living, memory care, and independent living) helps build brand credibility. A Clinical Nurse Liaison should present services with warmth, professionalism, and a genuine commitment to quality care.
  • Efficiency and Organizational Skills: Managing multiple referrals, follow-ups, and documentation tasks requires a high level of organization and time management. Being able to multitask efficiently, especially when coordinating closely with the admissions team, accelerates the admission process and keeps census flowing steadily.

Requirements

  • Possession of an active and valid Georgia RN license which is in good standing OR an active and valid unencumbered multistate nursing license that is in good standing
  • Ability to meet all health requirements, including a drug screen.
  • Ability to pass a criminal background check.
  • Prior long-term care experience is preferred but not required.
